Bifurcational lesions The first set of lesions we would like to discuss is bifurcational lesions. As shown in slide 26, the first consideration when dealing with a bifurcational lesion is to ask ourselves: are we going to use one stent or two?  Slide 26 shows a series of questions for which the answer will condition the operator more towards one stent versus/or two stents.  If the side branch is only large, but not diseased, we may stay with one stent.  It is important, as said in slide 27, to reevaluate the need for two stents after predilatation of both the main branch and the side branch.

The fact that the probing catheter is advanced over the wire coaxial to the stent deployed in the main branch will also prevent the second wire from engaging below the struts of the mainbranch stent.  The inflation pressure should also be kept well under the rated burst pressure because balloon rupture within a stent strut can also cause balloon entrapment.  Finally, kissing balloon inflations in both the stented main branch and in the side branch should liberally be performed because stent deformation following sidebranch dilatation might have occurred.
